Our Parent Handbook covers all Creative Kids policies and procedures, and is a good read for parents new to the program, and those who have been with us since the beginning. It covers our Admission Requirements, Registration Information, and any applicable discounts, as well as our general policies regarding employees, food, and conduct. Health Appraisal Form
The Health Appraisal form gives us the relevant health informtaion regarding your child. It is important that if your child has any special needs like athsma or allergies, that you fill out this form accurately when you enroll your child - and it is State law that we have this form within thirty (30) days after your child’s enrollment. The form must be signed by your child’s physician. Get a copy of the form from the link below.

Child Emergency Card
The Child Emergency Card is kept on file should an emergency arrise. It informs us of the key contacts for your child, so that should anything happen we can contact the correct people immediately. These cards are available to us in the office, and they accompany us should we go on field trips as well, so your information is always safe and at hand when needed. Download a copy below.
